Coach Evaluation
Please complete the following COACH evaluation form.
Your response and suggestions are very important and are always used to improve the quality of our baseball program.
You are not required to fill in your name or phone number but it is useful if FLBA feels the need to get follow up information from you.
If you have concerns or suggestions that you would like to discuss with FLBA, please contact the Board Chair. This should be done immediately after the season ends.
Coach evaluations are only seen by the Board Chair and then passed on to the Coach Selection committee for Coach selection purposes the following year.
Coaches DO receive a copy of all evaluations without your personal information.
Please, one response form per family.
